Evaluation Process

​The process begins with reviewing potential candidates based on their needs-based application. A certified mobility specialist, physical therapist, occasional therapist or case manager will be involved prior to deciding on the mobility equipment. CAP-C, Innovation, and VA grant holders are welcomed, but each recipient is based on the needs-based assessment. These evaluations will be conducted in person or by video chat. Once the proper solution has been determined, Y Me Mobility Assistance will provide the funding to assist the candidate, which is payable directly to the vendor. 

Please note: everyone may not qualify. Also, a doctor’s prescription is required for the mobility equipment prior to delivery.
